You can use Schedule2.IT’s integrated DEA Form 41 to electronically fill out your DEA Form 41 (Registrant of Controlled Substances Destroyed) as required by the Drug Enforcement Administration when self destroying your expired or damaged drugs.
Before completing this form, you must first place your expired/damaged medications into the Quarantine Inventory.
1) Navigate to Forms and select DEA Form 41
2) Click on "Add DEA Form"
3) Complete the following sections of the form
Section A.
Section B.
Section C.
Section D.
4) Print Form (Optional)
